Catholic Healthcare West
CHW decided that with over 1,500 employees needing to fill out the Annual
Conflict of Interest form, that putting it on-line would be more efficient and
less costly than doing it by hand this year. Using the FormDriver and FormBuilder
technologies, We Do That! Technologies (WDTT) was able to build and put this on-line in
less than 10 hours. Employees fill out the Dynamic HTML page, submits data to
the database, notifies the Administrator, then the Administrator marks off
the list the forms as they get them with the "wet" signature. WDTT then exports
the data for CHW to import into their own monitoring and analysis system.

i1003 Systems
The Uniform Residencial Loan Application (form 1003 for Fannie Mae) is a form
that every purchase or refinance will need to fill out. Using the FormDriver
in conjunction with the FormBuilder Technologies, we were able to
create all 9 sections of the 1003 form plus the Credit Application (pdf),
Upfront Documents (pdf), Final 1003 (pdf), the Good Faith Estimate (html, print in pdf),
an HTML menu to drive the entire system, Broker log in and sign up pages,
Client log in and sign up pages,
and Reports generation for the Brokers all in less than 40 hours.

Artwork Media is a company with a unique approach to e-Brochures that is bound to
catch on. Working with companies to produce PDF e-Brochures for direct sales
giving their customers top quality e-Brochures to schedule meetings, order products,
or become hosts, now is made possible with the FormDriver Technology processing the
information and data that comes from these e-Brochures.

United Methodist Church Florida Conference
The UMC Fla Conference uses the FormDriver Technology to process data for their
year-end Pastor, Church, and Status profiles. Utilizing the built-in Registration
and PIN generation adds security. The Conference is comprised of some 750 + Pastors
and 750 + Churches. This is their 3rd year using the system.

Trent University Petersborough, Canada
Students coming to stay in the dorms can order their telephone service on-line
with the system built by We Do That! Technologies. A student goes to the "Sign up"
page, fills in their necessary information, then the Communications director gets
an e-mail notifying them that a student has signed up. The Comm Director then
generates a report using the FormDriver Technology to show students that need to
have dorm and rooms assigned, the system then looks up in the telephone database
to retreive the telephone number for that dorm and room, pre-fills all student
data and the new phone information into an HTML form that the Director looks over
and approves, upon submission, the FDT fills out the Bell Canada pdf form and
e-mails it as an attachment to the Bell Canada phone company representative.

OAO Corporation/NASA's Jet Propulsion Laboratory
Converted OAO's paper-based Category A timecards to a web-based
timecard for submission and extraction, updating their Billing
Program with a secondary output that interfaces with JPL's Oracle
system. That system ran for 39 months without ever needing to call us
for support or upgrades before their contract ended.

Major Entertainment Company
PDF Artwork Approval and Marketing forms were sent in via e-mail from companies
all over the world. The FormDriver Technology extracted the data into a database,
then incremented the counter for that entry and wrote it back into the PDF form.
The system became 100% paperless, with status being on-line and review of artwork
also being done on-line. Upon approval of the artwork, updates and comments where
written back into the pdf form and e-mailed back to the Originator.

Answer Financial
Insurance Brokerage company with about 8 insurance companies
participating in their HealthCare line of business. FormDriver
takes the XML output from the ASP inputs and populates the correct
state and carrier with the web-submitted information for almost 300 pdf forms.

Performed a Proof-of-Concept using FormDriver to extract data
submitted for the Annual Budget for the City of Los Angeles. Using
Crystal Reports to generate the final report for analysis.

Doctor Enrollment form using PIN generation that allows for Diagnosis
forms to be pre-filled with Physician data and updated with Patient
data.

Third Party Administrative Claims company that uses Acord forms
for Property Loss, Auto Loss, General Liability, and Worker's
Comp. Data is entered via the web and returns a savable copy of
the pdf form to the End User.

City of San Luis Obispo
Uses FormDriver to take Job Applications on-line. An Applicant
inputs the data into HTML app., submits it and a copy is pre-filled
and e-mailed directly to the Personnel Department in PDF format. The system also
generates a report that allows the HR director to pull up PDF W4 and/or I9 forms
already partially filled with the applicant's data to be finished and printed.
